Good Afternoon,

About two weeks ago, we sent you an email asking you to participate in an online survey concerning your job as a healthcare worker.  This survey is being conducted by the National Institute for Occupational Safety and Health (NIOSH).  NPTA has agreed to make the survey available to our members as we are very interested in any data which can lead to improved health and safety of our members. 

If you have not already completed the survey, please do so today. The survey will take approximately 30 minutes to complete.

While your participation is valued and very important for the success of this effort, your choice to participate is completely voluntary. Your responses to all questions will be kept secure.  No personal identifiers will be recorded.  

Survey Instructions

1.       Note your organization key: NPTA       You will need this to access the survey.

2.       If you need to, you will be able to exit and reenter the survey where you stopped within 48 hours. Because many of the questions are time-sensitive, if you return after 48 hours, you will be asked to begin the survey again.
